We led Form's seed to build the software backbone healthcare's long tail actually runs on.
The company
Form is vertical SaaS for independent medical and dental clinics — scheduling, records, billing, and embedded payments in one system built for a market horizontal tools never served well.
Our thesis
Our thesis: own the system of record for a neglected vertical, then layer payments and financing on top. Form's founders knew the clinic workflow cold, and the switching costs of being the source of truth make revenue extraordinarily sticky.
The outcome
Form reached 1,200 clinics and 62% of revenue now comes from embedded payments. Net revenue retention sits at 148%. We led the seed and joined the board.
